"Getting You There" - A Transition Planning Toolkit

Introduction

Getting You There is a program designed to assist you as well as other teens and young adults who have spina bifida and/or hydrocephalus (sb/h) with transitions. In many ways, your experiences will be similar to most youth without disabilities, but there are important differences that create special challenges. Most youth for example, are eager to become independent, to have the freedom to shape their own lives. You may feel the same way but might face some obstacles like being more dependent on parents and other family members. Regardless of your abilities, you have a strong need to be like other young people your age and to do the same things to fit in.

Our Getting You There Transition Toolkit  contains tools to guide and assist you as you work to achieve your personal goals. These tools were designed to help you to understand, prepare and make your transition into adult centered health care, post secondary education, employment, independent living and to assist you in developing your social network.

Planning for the big changes in your life as you grow up is important for any young person. It is even more important for youth with spinabifida and/or hydrocephalus. You simply have more details to manage and a lot more needs to be in place for you to be successful. 

Use this transition toolkit to help you develop and work a plan for yourself. Follow it from beginning to end, or just use the pieces that you need.
